These surveys help me to best serve you and are strongly recommended if you have any dietary restrictions or other needed accommodations. Your responses are private and will only be shared with me. Please try to fill them out 24 hours+ prior to your first service. You can fill out or edit preference surveys at any time.

WAIVERS:
You may receive an email with a waiver for in-home services after booking. Please fill out the requested waiver 24 hours+ ahead of the service date.

For any requested deposits for services, you can utilize the links below. Please wait for the final payment until I have contacted you with an invoice total. If you need to pay with cash, check, bank transfer, or money order please get in touch with me to coordinate.
